ABSTRACT Background: Corticosteroids are the mainstay of treatment of moderate-severe ulcerative colitis (UC). However, very few studies have analysed the short term clinical outcome (maximum 1 year); even fewer have evaluated the endoscopic response and its prognostic role, and none in newly-diagnosed patients. Aims: To prospectively investigate the clinical and endoscopic outcome after the first corticosteroid course in a hospital-based inception cohort of newly diagnosed UC patients. Patients and methods: A cohort of 137 patients (M/F 96/41, mean age 36.2 years) with active moderate-severe UC needing their first systemic steroid therapy within 12 months (median 1 month) of diagnosis were prospectively evaluated according to clinical [Powel-Tuck (PT)] and endoscopic [Baron (Ba)] indexes at 3 and 6 months and then every 6 months for 5 years.
